Medication Therapy Management (MTM) is a structured, one-on-one consultation with a licensed pharmacist who reviews everything you take and looks for interactions, duplications, gaps in therapy, and opportunities to save money. Many Medicare Part D plans cover MTM at no cost to the patient.

What happens in a review

Plan for 20 to 30 minutes, in person or by phone. Bring every bottle you have — including vitamins and anything you bought over the counter.

  • Comprehensive medication review of prescriptions, OTC products, and supplements
  • Screening for drug-drug, drug-food, and drug-condition interactions
  • Identification of duplicate therapy and unnecessary medications
  • Lower-cost therapeutic alternatives and generic substitution options
  • A written personal medication list you can share with any provider
  • A written action plan with clear, plain-language next steps

Working with your prescriber

If we find something that needs attention, we contact your prescriber directly with a documented recommendation. You are never left to relay a clinical message yourself. We follow up until the issue is resolved.

Coverage

Medicare Part D beneficiaries who meet plan criteria typically receive MTM at no cost. Many commercial plans also cover it. If yours does not, we still offer the review — call us and we'll explain your options before anything is scheduled.

Frequently asked questions

How often should I have an MTM review?

At least once a year, and any time your therapy changes significantly — a new diagnosis, a hospital discharge, or a new prescriber.
